TITLE

Disposables: vent valves

DETAILS OF INCIDENT

The venting valves are built into the cardiotomy suckers/cardiac vent. The green valve failed the preoperative wet sucker test, and the remaining red sucker and blue vent failed during the procedure.

DETAILS OF INJURY TO PATIENT

No harm to patient.

ACTION TAKEN

We had to cut out the affected valve on the vent and insert a 1/4" connector and vent with a needle. Patient's direct care was not compromised. Reported to company representative.
